(To be completed by parent or guardian and adults registering for a youth camp week as an advisor) When registering for camp the grade in school refers to the current grade a child is in and will complete in May of 2012.
Group Registration
(To be completed by Pastor or youth leader/director) When registering for camp the grade in school refers to the current grade a child is in and will complete in May of 2012.
Note: Group registration is to be used for two or more campers attending the same LO summer program and week. To complete group registration you will need to fill out name, birthdate, grade, mailing address, parent/guardian name, phone number, and email for each camper.
Specialty Week Registration
